Enter dental lab and patient information

Feature Description

  1. This section is used to select the ordering dental clinic and enter staff and patient information before creating the request form.
  2. If the dental lab creates the request form, please enter the dental clinic information.
  3. Patient name is required. Chart number is optional, but entering it is recommended.

How to Use

  1. Select the dental lab or dental clinic.
  2. Select the staff member or assigned dental hygienist.
  3. Enter the patient name and chart number.

TIP

  1. Entering the chart number makes it easier to identify the same patient and search orders later.
  2. If the desired dental lab or dental clinic is not displayed, first check the client status in Affiliation Management.
  3. Request forms created for unregistered dental clinics are not linked to dental clinic accounts on the platform, so they are managed separately as internal management clients.
Select data type and completion date

Feature Description

  1. Select the data delivery method as IMP or SCAN.
  2. The order date is set based on the day the order is created. If the dental lab separately enters a request form for an unregistered dental clinic, please enter the actual order date.
  3. The completion date can be adjusted to match the delivery or production completion schedule.

How to Use

  1. Select IMP when proceeding with physical delivery.
  2. Select SCAN when attaching digital files such as scan files.
  3. If the completion date does not match the actual delivery schedule, edit the date.

TIP

  1. The order date and submission date may be different, so enter the actual order date accurately.

  2. Select Remake when remake work is required.
